Login Instructions:

For Your First Login
Your Username is:
The first initial of your last name, then Member number for the primary Member
The first initial of your last name, Member number followed by A for the spouse Member
Example: Primary S4321, Spouse S4321A

Your Password is:
Your last name
Example: Smith
*If you do have a suffix please use this as part of your last name login for the first time (Such as Smith, Jr.  be sure to add a comma, a space, and a period after Jr.)
After logging in, you will be prompted to change your Username and Password to something more secure.
This will become your permanent Username and Password.
